Starting Chronic Opioid Therapy

  • Review and sign pain agreement with patient. (Form available under Letters tab in EPIC).

    • Place in the scan bin after its been signed and give a copy to patient.
    • Add "Pain medication agreement signed" to the problem list (ICD V58.69).
    • Order a urine drug test: Monitor Drug Profile 10 or 14
  • Check iStop.

Continuing Chronic Opioid Therapy

  • Ensure there is an updated patient-provider agreement annually to review medical indications for opioid therapy and benefits versus risks.
  • Patients must be seen at a minimum every 3 months in person.
  • Remember to check urine drug tests at least every 3 months.
