What is Palliative Care at Home?
A new program to care for patients who wish to be at home and focus on comfort and quality of life. It is open to patients living in the 5 boroughs of NYC. Staffed 24 hours a day, 7 days a week by physicians and nurse practitioners who will perform video or telephone visits.
What are the criteria for referrals?
Patient must live in Manhattan or Queens and have HealthFirst insurance with a Mount Sinai PCP.
