If you have patients who are not currently in the office but that you would like to be scheduled, email the supervisor, Zak Shinaba and he will have their staff reach out to your patients to schedule an appointment. NOTE: the clinic accepts Medicaid, Medicare, and some commercial insurances. If patients have a commercial plan not accepted by the clinic, they will offer to schedule the patient at the faculty practice on 85th Street.
