Criteria: Patients with persistent signs, symptoms, or conditions after a confirmed or presumed COVID-19 infection that have lasted at least 3 months.
Sessions: Firm E, Wednesday AM and Thursday PM
Epic: Place referral to "AMB REF to Center for POST-COVID CARE" and select 17 E 102nd IMA 3rd Fl as Department
Clinic: Select "IMA Long COVID Clinic" on the .checkout sheet for the front desk to schedule the appointment before they leave.
