Indications: Risk of patient hurting self (active SI with intent and/or plan) or others (active HI or psychosis with aggressive/violent thoughts) OR patient unwilling to comply with recommendations (such as a voluntary evaluation / admission to psych).
Mon-Fri 9-4pm call triage SW at 212-824-7583 (x5-7583) to wait with patent until security arrives. After 4pm or on Saturday have an MA or nursing staff member wait with patient.
Call (or ask front desk to call) CAM security at 212-824-7005 (x5-7005) to wait with the patient.
Patients must be evaluated by an attending physician.
Complete suicide risk assessment (.suicideimascreen) OR the violence risk assessment in docflowsheets.
Call the Psych ED to review the case 212-241-5637 (x4-5637)
If Psych ED evaluation needed – call 911 for ambulance and police for transport.
If patient at risk for elopement wait until security and/or police on the scene to inform patient.
Call the ED referral line 212-241-3611 (x4-3611) to notify that the patient is coming and will need a 1 to 1 observation while being triaged before going to the Psych ED for evaluation.
If imminently dangerous patient attempts to leave clinic, do NOT physically try to restrain patient. Instead call 911. If the patient does leave the clinic you can also call Mobile Crisis (888) 692-9355 or visit the website nyc.cityofnewyork.us and enter the information.
You may also contact the Psych ED attending to discuss SAFE Act reporting if you believe the patient is in imminent danger of harming themselves or others.
