Patients who are looking for long term Home Care and have medicaid will have to call New York Independent Assessors at 1-855-222-8350 to schedule a home visit. Before services can begin a DOH form (M11Q or CDPAP) will need to be filled out by a provider. Social work can only fill out renewals if there is a prior form on file in the media section. DOH forms must be completed within 30 days of a visit.
Only medicaid covers long term unskilled services. Medicare and commercial plans do not. For patients without medicaid or need further assistance, please refer to social work or REAP.
