This clinic is meant to serve as a hypertension "boot camp" with in-person, video, or telephone visits every 2-3 weeks until the patient achieves control. Please refer patients with uncontrolled blood pressure for hypertension education, medication management, medication adherence counseling, and/or home BP teaching.
Epic: Please complete the "IMA Hypertension" referral form
Clinic: Select "IMA HTN" on the checkout sheet for the patient to schedule an appointment before they leave.
