Located at 440 West 114th Street at Mount Sinai Morningside
Inclusion criteria: Persons aged 65 and older reporting 2 or more falls in last 12 months OR a single fall with injury requiring treatment. Patients must be living independently or in an assisted living facility.
Exclusion criteria: Nursing home resident, acute injury requiring long-term rehabilitation such as hip fracture, or severe dementia.

Please include: The best phone number and person to contact about appointment, where you are referring from, the PCP of the patient, and any other relevant information you think might be helpful (about the fall(s) or in general).
