Criteria: Patients with persistent symptoms after a confirmed or presumed COVID-19 infection that have lasted at least 4 weeks and cannot be explained by an alternate diagnosis.
Sessions: Firm E, Wednesday AM and Thursday PM
Epic: Place referral to "AMB REF to IMA Long COVID Clinic"
Clinic: Select "IMA Long COVID Clinic" on the .checkout sheet for the front desk to schedule the appointment before they leave.
